Registering with the Practice
Please ask at Reception for the forms to complete to register at the Practice. Please bring along your medical card if possible, so that we can record your unique NHS number and use this to request your health records form your previous practice. We will then invite you to a new patient check at the surgery. This will involve collecting baseline information eg weight, height, smoking habits, exercise and dietary habits.

Other Services
Hearing Aid Clinic - for repairs/ adjustments to your existing hearing aid. The next clinic is 4th March 2011at Coltishall Surgery, please contact reception for appointment. Clinics are held every 2 months.
Sick Certification/ Statement of Fitness for Work
From the 6th April 2010 the Doctor will issue a statement of fitness for work if you have been absent from work for more than 7 days. If it is less than 7 days you will need to complete the self-certification form provided by your employer. If a certificate is required for less than 7 days a charge is payable.
